This is what to expect when you use our self referral portal.

Submit a self referral using the form on this page.
2.
We will schedule a time for you to meet with one of our Peer Support Coordinators who will help you through the registration and assessment process.
3.
Work with your Peer Support Coordinator to decide the level of support you need and action a care plan. This could include more information on our support groups, benefit advice or counselling.
All of our services will be outlined, and we’ll do our best to ensure you have a positive experience.
